On its website quebec.ca , the government of Quebec has posted a self-assessment tool for symptoms of COVID-19 We invite you to complete this document in order to obtain a recommendation on the next steps to follow, according to your condition.
Screening criteria
6 months to 5 years old
If your child has any of the symptoms listed in this list, it is recommended that you keep your child at home and avoid contact with other people.

Adults
All people 18 years of age and older with at least one of the following symptoms:
- fever (oral temperature 38,1 ° C (100.6 ° F) and above;
- sudden loss of smell without nasal obstruction with or without loss of taste
- recent cough or worsening chronic cough;
- shortness of breath;
- difficulty in breathing;
- Sore throat;
- runny or stuffy nose.
Or two of the following symptoms:
- nausea (heartache) or vomiting;
- diarrhea;
- intense fatigue;
- significant loss of appetite;
- generalized muscle pain (not related to physical exertion);
- headache.
People who have difficulty breathing (shortness of breath, feeling of suffocation) OR who have severe breathing difficulties even at rest (eg, shortness of breath that prevents you from speaking) should call 911.
For person with symptoms with a planned appointment
Designated assessment centres (DACs):
Required symptoms:
- Requires primary health care that cannot be provided over the phone by their family physician or a primary-care nurse practitioner (PCNP).
AND
- Shows symptoms of influenza-like illness (ILI) or COVID-19.
To get an appointment:
- Contact your doctor's clinic or call 1-877-644-4545. An agent will transfer you to the COVID-19 call center in Estrie.
- Your condition will be assessed over the phone and you will be given an appointment at a DAC, if indicated.

Prioritization of testing
- people who have been asked by public health authorities to get tested;
- Moderate- to high-risk close contacts who are symptom-free should be tested only once during their 14-day period, approximately on day 6 from the last contact. Isolation continues even if the test is negative.
- Contacts should be tested if they develop symptoms (even if they have already been tested and were negative).
- Individuals who have symptoms of COVID-19 and who, as a result of a call to the COVID-19 information line or taking Self-Assessment Tool, have received a recommendation to get tested.
If you don't have symptoms, if you have'nt been in close contactwith someone who has COVID‑19,or if Public Health has not contacted you, it is not recommended that you get tested.
Groups not prioritized for testing
- Individuals with COVID-19 who have already tested positive for it. Retesting to confirm the end of your isolation is not recommended.
- "Contacts of contacts": Individuals in close contact with another person who is in isolation because they have been in close contact with a COVID-19 positive case.
- Individuals who were notified about the COVID Alert application but who have not been contacted by Public Health.
Individuals who have tested positive for COVID-19 within the past three months and who are symptomatic again should call 1-877-644-4545 for an appointment at a designated assessment clinic. They must not go to a designated screening centre.

Where to get tested
Visit the testing location closest to your home, as the testing capacity is adapted to the local population. For example, you may be denied access to a testing location outside of your local service network (RLS).
Please bring your valid health insurance card and / or hospital card.
IMPORTANT: Be sure to give your address in the Eastern Townships (Estrie) when taking the screening test.
Walk-in (no appointment)
Reduce your waiting time: register your information in advance in our system to take a screening test without making an appointment!

- Go to a designated walk-in screening centre.
- Tell the person at the reception desk that you have completed your registration form for walk-in screening.
