If you are waiting for an appointment in the upcoming months but know that you will be unavailable for an extended period.
Examples: vacation, travel abroad, etc.
By providing this information, your appointment date will be scheduled accordingly.
IMPORTANT | If you already have an appointment scheduled during the period you will be unavailable, it will be rescheduled. You will receive a new appointment date. Please note that in accordance with MSSS regulations, your name will be removed from the waiting list after the third (3rd) postponement or after 2 unexplained absences, and that requesting to reschedule on the same day as your appointment will be considered an absence because such short notice does not give the appointment centre sufficient time to assign your time slot to another patient.
This procedure will take 5 minutes of your time.
You will need to provide the following information:
- your medical insurance number (on your sun card)
- your hospital card or your date of birth
- a valid e-mail address
The above information will help us to identify you and to ensure you are the correct person.
